Dr. Kornely is a huge college basketball fan (especially for her Purdue Boilermakers) and would like to get you involved in March Madness with her.

Pick the winning team of the NCAA Men's basketball tournament AND the total combined score of the final championship game on Monday, April 3rd to win a prize!

The 4 contestants who pick the winner and come closest to the total combined points scored by both teams in the championship game win a prize.
First place prize is a $100 VISA gift card.
Second place prize is a $50 Let's Eat gift card.

(Panera, Red Lobster, Cracker Barrel, Cheesecake Factory, Chili's)

Third place prize is a $25 Kohl's gift card.

Fourth place prize is a $10 McDonald's gift card.

The gift cards can either be picked up at the office or mailed.
The winners will be contacted the week of April 3rd.
Only one entry allowed per person.

Submit your entries via our website, www.footclinicofwestbend.com.
In the "Contact Us" tab, enter your full name and email address, type NCAA as the subject, and in the message area, enter your pick for the winning team AND the total combined points scored by both teams in the championship game along with your phone number. Then click Submit.

Only submissions that come from our website will be valid entries.

You do not have to be a current patient to win.
Submissions must be received by 2:00 pm on Thursday, March 16.
